Remote Patient Monitoring (RPM)
Bluetooth devices (e.g., blood pressure cuffs, scales, glucose monitors) automatically send readings to our care team for review and early intervention.
Transitional Care Management (TCM)
After a hospital or rehab discharge, our team calls within 48-72 hours to ensure a safe recovery and prevent readmission
